Must have active Medicaid. 11 round trips per calendar year covered
Transportation to medical, dental, mental health appointments only
Medicare Patients- no wheelchair transports for medical appointments. No physician office transportation
Transport to and from hospital (non-emergency) if bed-confined
Medicaid Patients- wheelchair transports ONLY IF cannot walk or pivot
Physician office transportation if bed-confined
Private Pay- call for wheelchair transport fees
